Best Practices in Homecare and Hospice Awards

About the awards...

PHA's Best Practices in Homecare and Hospice program promotes best practices among home health, hospice and homecare agencies serving patients and consumers in Pennsylvania. The program advances the quality of in-home services through the exchange of ideas among peers.

Eligibility...

Open to all agencies that offer home health, hospice and/or homecare services in Pennsylvania. Entry is free for PHA members. The non-member fee is $35 per entry.

Evaluation...

Entries will be judged in two phases: peer review by a panel of past Best Practices winners and PHA members followed by phone interviews with a panel of state officials and healthcare experts.

Entries must effectively demonstrate:
• Process and/or planning
• Staff involvement
• Measured improvement
• An ability to be replicated

All entries are scored using an objective rubric.
