Pennsylvania Foundation for Home Care and Hospice

The Pennsylvania Foundation for Home Care and Hospice is a non-profit organization created to promote and support home health, home care, and hospice services in Pennsylvania.

The Foundation promotes community awareness, supports the provision of in-home services, and provides financial assistance for customers of home care, home health, and hospice. In doing so, the Foundation makes these needed services more accessible and affordable to our communities.

The Foundation further promotes quality care through the enhancement of career paths for the home health care workforce, including education, recognition programs, and more.

The Foundation provides short-term financial assistance to support qualifying individuals in need of home-based care services through the Home Care Grant and South Central Pennsylvania Home Care Grant programs.

NEW! South Central Pennsylvania Home Care Grant

The Pennsylvania Foundation for Home Care and Hospice, through funding provided by the Partnership for Better Health, has established the South Central Pennsylvania Home Care Grant.

This grant is available to all home-based care providers that provide services to individuals in the eligible zip codes. There is no limit on the number of applications that can be submitted per agency; however, only one application per client/consumer will be accepted. View the Grant Flyer for more details.

Review the South Central Pennsylvania Grant Application Packet for more information or to apply. Please note: Once an eligible agency has been approved for a Home Care Grant, the agency must provide service and invoice PHA to receive the grant proceeds. Home Care Grant

The Home Care Grant supports home-based care agencies in providing temporary care to individuals who demonstrate a need for home and community-based services, but are not yet receiving or eligible for services through other payment programs, such as Medicaid. Review the Agency and Client Application Form for more information or to apply. Please note: Once an eligible agency has been approved for a Home Care Grant, the agency must provide service and invoice PHA to receive the grant proceeds. United Health Foundation Awards $750,000 Grant; Funds Used to Source PPE and Develop COVID-19 Education for Home-Based Care Providers

In 2020, the Pennsylvania Foundation for Home Care and Hospice received a $750,000 grant to support home-based care providers and workers in responding to COVID-19 in Pennsylvania. Through this grant, the foundation was able to purchase and distribute critically needed PPE to home-based care providers across the Commonwealth. Additionally, grant funding was used to develop seven COVID-19 specific training videos for My Learning Center.
